If you have an open records request, you may either:
1) Fill out the form below and submit on-line, OR
2) Print out the WORD or PDF form at the bottom of this page, fill out and return to Kewaskum Police Department via in person, mail, fax or email.
Comments:
- Please allow 5 to 10 working days for your request to be processed.
- You will be contacted when the report is ready.

The Wisconsin Division of Motor Vehicles will also provide a copy of the accident report (MV4000/MV4002) for $6.00 by visiting their website.
